We are excited to announce our Online Registration Process for 2017-2018. Listed below are the requirements for student registration. This year’s registration fee is $100 per student. Parents/guardians completing the registration process including the online registration process, returning all required forms, and providing proof of residency prior to July 1, 2017 will be eligible for an early registration fee of $50. 

Proof Of Residency Requirements

Per the Administrative Procedures for Board Policy 7:160 Student Residency, all families must provide proof of residency within the district. All Parents/Guardians will need to complete the following forms with proof of residency. You can return your registration packet information including your proof of residency documentation two ways: (1) In the provided envelope return the completed paperwork/documentation OR (2) Return all completed paperwork/documentation to the Main Office.

The Main Office and Administrative Staff will be available to assist with online registration and proof of residency starting on Wednesday, April 5 until Thursday, June 7 from 9:00 a.m. - 3:30 p.m. We will also be offering assistance after school hours on the following dates and times:

May 16                  4:00 p.m. – 6:00 p.m.  

May 18                 4:00 p.m. -  5:30 p.m.

June 14                 4:00 p.m. – 7:00 p.m.

Parents/Guardians that own or rent in Hillside School District 93

  1. Complete the Online Registration Process via Infinite Campus
  2. Complete the Residency Form and provide proof of residency documentation (leases must be notarized)
  3. Complete the Student Yearly Health Information Sheet  
  4. Complete and sign the acknowledgment sheet
  5. Pay the Registration Fee via Infinite Campus  

Parents/Guardians that lives with another person residing in Hillside School District 93 (Please contact the school to obtain the additional form in bold type)

  1. Complete the Online Registration Process via Infinite Campus
  2. Complete the Residency Form and provide proof of residency documentation
  3. Complete the Student Residency Affidavit- Resident must provide proof of residency (notarization required)
  4. Complete the Student Yearly Health Information Sheet
  5. Complete and sign the acknowledgment sheet
  6. Pay the Registration Fee via Infinite Campus 

Parents/Guardians of student residing with another person who has been given custody of the student and Resides in the District (Please contact the school to obtain the additional forms in bold type)

  1. Complete the Online Registration Process via Infinite Campus
  2. Complete the Residency Form and provide proof of residency documentation
  3. Complete the Student Residency Affidavit
    1. Parent/Guardian of student residing with another person who has been given custody and resides in the district (notarization required)
    2. Person (Non-Parent) who asserts that student lives with him/her within the district (notarization required)
  4. Complete the Student Yearly Health Information Sheet
  5. Complete and sign the acknowledgment sheet
  6. Pay the Registration Fee via Infinite Campus 

 *The state of Illinois requires all students entering sixth grade have the following medical requirements on file: Current school physical; Proof of current T-dap vaccine; Proof of two varicella (chicken pox) vaccine; Proof of MCV4 (meningitis vaccine); and Current Dental exam. All students entering second grade require a current dental exam. 

Verification of Residency: You must provide ONE (1) document from Category I and TWO (2) documents from either Category II or III. 


Category A: ONE of the Following Documents
 Current Real Estate Tax Bill
 Current Mortgage Papers
 Current Signed Lease (Lessee of the unit listed) AND letter from the Landlord listing occupants of the unit [notarized]
 Letter of Residence from the Landlord listing occupants of the unit in lieu of lease [notarized]
 Letter from Manager and proof of Last Month’s Payment, e.g., canceled check or receipt [notarized]

AND
Category II: ONE and/or TWO
of the Following Documents Showing Your Name, Address and Current Month or Year
 Current Illinois Driver’s License
 Current Illinois State ID Card
 Current Illinois Voter Registration Card
 Current Home / Apartment Insurance
 Current Illinois Medical Card
 Current Automobile Registration
 Current Major Credit Card Bill
 Current Checking or Savings Account Statement
 Current Public Aid Card

Category III: ONE and/or TWO of the Following Documents Showing Your Name, Address and Current Month or Year [dated within the last 30 days]:
 Gas Bill
 Electric Bill
 Water Bill
Cellular and home telephone bills will not be accepted.

Bus Assignments: Please check the transportation link on the district’s website over the summer for updates about bus transportation.
